    There is evidence that variability exists in pediatric subspecialty use, and it can be difficult for primary and specialty care providers to collaboratively manage patient care.  In an effort to improve collaboration and co-management, the MMP Pediatric Specialists have created over 30 referral guidelines that provide both education and decision-support for common conditions.  In this webinar, Dr. Mike Dedekian will present two referral guideline examples related to pediatric obesity and diabetes.  By the end, you will have an understanding on how to read and best utilize the guidelines, when to refer, as well as how you can co-manage your patient’s condition with their pediatric specialist.

    POLST, which stands for Physician Orders for Life Sustaining Treatment, is a type of advance directive that patients who may have less than a year to live can use to inform providers about their treatment wishes. Dr. Rob Hunold had read numerous studies on the impact of POLST, and discovered that it had a major positive impact on patient outcomes and cost. So, he set out on a campaign to increase the number of patients in his practice who had completed POLST forms. He pulled it off, but only with perseverance and personal sacrifice.
